This article investigates the facilitators and barriers to implementing hospital-based health technology assessment (HB-HTA) in hospitals across seven countries. Using a two-round Delphi study with 15 HTA specialists from France, Hungary, Italy, Kazakhstan, Poland, Switzerland, and Ukraine, the study identifies key barriers and facilitators. Experts reviewed HB-HTA implementation in their countries, ranking barriers and facilitators on a seven-point Likert scale. Consensus was achieved on six out of 12 statements: two barriers and four facilitators. The main external barrier was the lack of formal recognition of HB-HTA in legislation, while the main internal barrier was the limited availability of dedicated human resources. The study suggests that while HB-HTA needs further development for formal acceptance, leveraging identified facilitators could help implement international strategies to strengthen HB-HTA practices.
