Outcome Harvesting Highlight: Primary Health Care Performance (PHC-PM) Activity
The PHC-PM Activity is transforming district health systems in Ghana and Rwanda. By equipping health management teams with leadership skills, data-driven decision-making tools, and catalytic grants, the initiative is driving measurable improvements in primary health care. An outcome harvesting evaluation was conducted to understand the project’s impact, which noted stronger leadership and teamwork, better use of health data for planning, and locally-led solutions that close service gaps. Together, these efforts are building more resilient, equitable health systems—advancing service delivery and improving population health.
